Abstract

UD Afareza Rajarak merupakan industri bergerak dalam manufaktur perdagangan eceran yang memproduksi rak UD Afareza Rajaraak terletak di Jl. Banjar Melati, Tlatah, Wotan Sari, Kec. Balongpanggang, Kabupaten Gresik, Jawa Timur 61173. Terdapat dua jenis produk yang sering kali dipesan oleh pelanggan yaitu rak dinding dan rak gawang. Terdapat 4 cacat produk seperti kawat melintang yang tidak presisi, besi hollow bengkok, kawat melintang bengkok, dan sisa pengelasan yang tidak bagus. Presentase cacat tertinggi untuk produk rak dinding sebesar 4,81% pada bulan Januari 2024 dan rak gawang sebesar 5,05% pada bulan Desember 2023. Menyebabkan kerugian omset pada UD. Afareza Rajarak disetiap tahunnya, dengan kerugian terbesar pada bulan Desember sebesar Rp3.581.250. Penelitian ini bertujuan mengetahui faktor-faktor penyebab cacat dan bagaimana cara mengurangi kecacatan produk di UD. Afareza Rajarak menggunakan metode Plan Do Check Act (PDCA) dan Failure Mode and Effects Analysis (FMEA). Faktor penyebab terjadinya kerusakan adalah faktor manusia, metode, mesin, bahan baku dan lingkungan. Perbaikan utama berdasarkan permasalahan yang ada yaitu membuat SOP pada mesin bor, menetapkan kebijakan penggantian produk cacat ke karyawan yang lalai saat proses produksi, merancang alat bantu pada proses bor, menambah lapisan kardus pada packing, menambahkan ganjalan kardus saat pengikatan saat pengiriman, melakukan pengecekan dan perawatan terhadap mesin yang sering mengakibatkan kecacatan, dan pembuatan check sheet.
