Abstract

The construction of library at UIN Sunan Ampel Surabaya is an effort to improve the quantity and quality of education services. Based on reports from the Library Construction service provider at UIN Sunan Ampel Surabaya, there was work accident during the construction. Therefore, Occupational Safety and Health (OSH) management is important in implementing every project. This is needed to reduce the number of work accidents, so that similar incidents do not recur in the next stage of the construction. The cause of the accident was determined systematically by using Fault Tree Analysis (FTA). FTA method was applied to determine the causal factors as well as the combination of causes that may lead to accidents. This research aims to (1) determine factors that may cause accidents on the Library Construction at UIN Sunan Ampel Surabaya, (2) determine how to handle and prevent accidents on the Library Construction at UIN Sunan Ampel Surabaya. After the data was analyzed, it can be concluded that the causal factors of accidents in implementing building construction project at UIN Sunan Ampel Surabaya can be divided into 2 categories: Unsafe actions such as inattentive,irresponsible, lack of security, ignoring PPE, lack of knowledge, lack of concentration, lack of training, dan stress. Unsafe conditions such as the position of dangerous equipment, pursued by production deadline, neglecting hygiene, lack of lighting, slippery, limited monitoring time, and limited time. Handling and prevention prior to work accidents are seen from two aspects,namely: (1) Management Aspects, arranging schedule of safety talk, safety meeting, safety induction, safety patrol, safety morning in the project environment. (2) Technical aspects, monitoring the effectiveness of repair and accident prevention.
