Abstract
PT. Angkada Raya is a manufacturing company engaged in the waste management of PVC, Vinyl hoses, which is located at Jalan Rangkah II No. 19 Tambaksari District, Surabaya City, in producing its products this company uses 3 main types of machines such as Grinding Machines, Plastic Oven Machines, and Injection Molding Machines, this company has 7 injection molding machines with old models and until now only 3 out of 7 can be used , to reduce downtime or prevent sudden damage, periodic checks are needed, if the injection machine suddenly cannot be used, it will have an impact on daily production capacity which is not met until there is a delay in delivery, therefore research is being carried out to find out what critical components are only what needs to be prioritized is using the Failure Mode Effect Analysis (FMEA) method, namely (RPN) with a total value of 606, as well as the proposed time interval for periodic checking or maintenance, namely Band heater 71 days, Mold 113 days, Screw 111 days, Cooling 57 days, CPU 152 days, Oil 122 Days and to find out the causes of frequent failures, the Fault Tree Analysis (FTA) method is used. There are 2 general factors, namely external and internal, such as Internal Band Heater, problems with wiring and external from raw materials and improper wiring, Internal Mold problems high temperature or cooling and external checking is not regular, Internal screw problems on the neck of the hopper overheat and external operators do not check regularly and the material is mixed with oil, runs out, freezes, Internal cooling has freezing and external lacks periodic checking, internal CPU overheating problems and external less periodic maintenance
