Abstract

Nabila Art Gallery is a industry manufacturingin the manufacture of aroma therapy lamps. The owner of this UKM is Mr.Pius, who employs 4 employees to run this business, to help run their business.This UKM uses 1 CNC machine wich is used for engraving on the lamp body. UKM Nabila Art Gallery is able to produce 40-60 units lamps/day. Continuous use of this machine causes the machine to experience trouble wich eventually disrupts the production process and event stop production. Maintenance on this CNC machine is very important. This study uses the Failure Mode And Effect Analysis (FMEA) method to identify the failure mode, and Logic Tree Analysis (LTA) to determine the consequence of failure caused by the failure mode and then determines the preventive maintenance action that must be carried out on the machine.From the result of the FMEA analysisobtained 7 failure modes consisting of the laser beam doesn’t come out, bearing ballscrew is loose, bearing ballscrew is worn, the engine movement is slow down, the optical lens is oxidized, the engine cannot start due to an error in the power supply and finally when there is a power outage. And then the result of the LTA analysis obtained 2 categories of failure modes, Economic Problem (C) and Outage Problem (B). Then the carry out is preventive maintenance actions according to their respective failure categories such as replacement sparepart, lubricantion, tightening ballscrew etc.
